It is incredibly gratifying to share the inspiring journey of grassroots groups as they turn small ideas and big dreams into meaningful realities through the support of Action for Healthy Communities’ Community Capacity Building Team (CCB).
Whether at a community event, a gathering among friends, or in the office, these initial connections spark deeper dialogues, engagement, and mobilization within communities. Our dedicated team nurtures these groups through mentorship, helping them identify their assets and needs, while providing critical feedback and tailored recommendations. This preparation empowers them to craft compelling Seed Grant Applications.
With the support of Seed Grant Funding, grassroots community groups embark on a journey toward making a positive impact in their neighborhoods. To further enhance capacity and foster networking opportunities, CCB organizes quarterly Communities of Practice (CoP). These gatherings are not just events; they are invaluable opportunities for community leaders to connect, learn from one another, and explore potential collaborations that amplify their collective impact.
Communities of Practice (CoP):
On December 6, 2025, we held the final CoP of the year, attended by 23 participants from 14 diverse communities. During this engaging 3-hour session, attendees reflected on the year, learned about other AHC programs such as Youth-Ready to Work and the Settlement – WRSP Program.
Participants took the time to share and reflect on the impact of their initiatives, proud moments, and the challenges they faced throughout the year. There was a truly heartwarming energy in the room as members shared their ‘win’ moments from the year. These weren’t just success stories; they were living proof of the positive ripple effect created by the Seed Grants and the tireless dedication of our community leaders.
Seed Grant Disbursement
This CoP was special because we also celebrated the approval of Seed Grants for seven new community initiative groups from the November 22 CDAC Meeting and disbursed the approved funds.
New Seed Grant recipients also received brief guidance on the implementation process, reporting, and evaluation of their initiatives, most of which are set to launch in February 2026.
As we look back on an incredible year, we continue to celebrate the resilience and creativity of our community partners. Together, we are building a brighter, more connected future.
